APPLY FOR A SERVICE DOG
Please read through the following information to determine if applying for a service dog from Freedom Service Dogs is right for you. In order to start the service dog application, you must first complete the pre-application questionnaire at the bottom of this page. Note: the approximate waiting time to receive a service dog is currently 2 – 3 years.

Freedom Service Dogs provides service dogs with full public access for:
- Individuals with mobility disabilities
- Veterans, first responders, and active-duty military with PTSD and/or traumatic brain injury
- Young adults with autism or other neurocognitive disabilities
We do not provide service dogs for:
- Seizure alert or response
- Allergy detection
- Diabetes alert or response
- Hearing or vision impairment
- Psychiatric support
- Tethering or tracking for elopement
- Individuals with dog allergies
FSD Service Dog application process:
- Complete the online application. Attach all supporting documents downloaded from the FSD website, including medical form, personal and professional references, and emergency contact consent. Submit!
- FSD Client Services Team reviews applications in the order they are received. Interviews are scheduled, either online or in person at the FSD facility. Support person is welcome to attend the interview.
- Submit background check. A completed background check for all adults in the household who are18 years of age and older is required.
- FSD Client Services team will notify applicant when a decision has been made. If accepted, applicant will be placed on the waitlist.
FSD will keep your entire application confidential. Your video/photo and written application will become the property of Freedom Service Dogs, Inc. FSD reserves the right to decline applications at any stage in the process if we do not feel you are a fit for our program.

Having a custom-trained service dog is a serious, long-term responsibility that is not appropriate for every person with a disability.
